    Combination of problems led to demise of P-36

    The National Petroleum Agency in Brazil says a combination of problems including poor design, maintenance and operational problems were the cause of the explosions and subsequent sinking of the rig P-36.

    A report produced by the Agency says faulty valves and tanks were at fault, as was a lack of training and supervision.

    In spite of the report, however, state-owned Petrobras has already received insurance payouts following the loss of the P-36.
